The relationship of free to total cholesterol has been examined in the serum of 819 African and Asian subjects in East Africa. With increasing levels of total cholesterol a constant relationship between free and total cholesterol values was not observed. The percentage of cholesterol in the free form increased from zero at 40 mg. per 100 ml. total cholesterol to 20.6 per cent at 300 mg. per 100 ml. We consider that in all studies in which total cholesterol is measured, the free fraction should be estimated and the result expressed in absolute amounts and not as a precentage of total cholesterol.
