"Dilution Anemia" Associated with Multiple Infusions of a Fat Emulsion

DAVID C. ZELLNER M.D.1 and JAMES M. IACONO PH.D.1

1 From the Division of Nutrition, Department of Internal Medicine, College of Medicine, University of Cincinnati, Cincinnati, Ohio

Intravenous infusions of fat emulsions in human subjects have been accompanied by decreases in the blood hematocrits. To determine why anemia develops five subjects were given consecutive daily infusions of Lipomul I.V. A decrease in the blood hematocrit was noted in all subjects. An increase in plasma volume occurred in all four of the subjects in whom it was measured while the circulating erythrocyte volume remained unchanged. Thus, the anemia produced was a "dilution anemia." The expanded plasma volume resulted from marked sodium and water retention which occurred during the infusion of Lipomul I.V. The reason for the sodium retention remains unexplained.
